Hulk doesn't seem to go all out when he's just annoyed. He smacked Spider-Man out of the way back when Spidey just learnt Mary Jane had apparently died on the plane crash, and Spidey's head was still attached. :)
It was interesting to see that Hulk stopped fighting when he overheard from Spidey ranting that "she can't be dead", and just let Spidey wear himself out

I saw him again in a Avengers comic (UK collection reprinting Avengers from 1-3 years ago), and the scientist in the group made a calculation that at the rate Nefaria was fighting (er, easily smashing ALL Avengers left and right of him), it would take around three weeks at the rate he was using power for it to drop a little.

Wasn't the US. Agent trained in the same style of combat as Captain America? Agility rises since Captain America trains in gymnastics, and as such, US. Agent did as well...
Incredible agility - maximum human potential, so, he's still human in that area...
Still, it is a little alarming that stat-wise, US Agent equals (or betters) Captain America in the physical skills department...

People with strength above Class 100 (the ability to lift/press 100 OR OVER) don't fit very well into the RPG. :)
Hulk, Juggernaut, and lots of others are all 'over' Class 100... Juggernaut, for example, his strength in the Handbook is reported as over Class 100 and 'immeasurable', as in, there was no known upper limit at the time.

Lady Death's stats would practically be a mix of the Death on here approaching Living Tribunal stats. :) She exists in every race and appears whenever someone is dying/dead, all at once. Reportedly, she will be the very last being in the universe when everything is gone. She'll put up the chairs on the tables, and turn off the lights and lock up. :)

In the comics... even at full strength, he was still defeated, but not by fighting means (body armor and regeneration usually prevented fast wins). When Hulk had banner intelligence, it seemed as if he was... stuck at Remarkable level, relying on his body and strength to pull him through?

That's what I used to think... if Thor's full might with his hammer was only able to SLIGHTLY dent true adamantium, it wouldn't work much on a being who's body is made completely from adamantium...
If I recall, many doubles of Ultron weren't made from adamantium, were they? Was that the case in this "Ultron Unlimited"?
